Heaton Medical Centre is home to an NHS general practice providing a comprehensive range of medical services within friendly, comfortable and modern purpose-built premises. There is a car park, easy access for the less mobile, with all consulting rooms on the ground floor, and facilities for disabled people and families with toddlers and babies. There is a pharmacy at the practice to dispense your prescriptions.

You will then be directed to information articles and videos advising you on how to treat your symptoms and what you may need to do next. The forms are quick and easy to use and tailored to give you help and advice that's specific to your condition. Service Status Due to unprecedented demand, we have taken the decision to temporarily disable the online consultation forms during weekend hours (Friday 18.30 - Monday 08.00).

If you have any urgent queries during this time, please contact the Out of Hours service on 0161 763 8940 or 111. You will be receiving a letter regarding Breast Screening in the post.
Services
We are currently accepting patients within the practice area.
Please check your postcode on our catchment area map, which can be found under Practice Information.
If you cannot provide us with your NHS number you must bring some form of photo ID to prove your identity, for example; driving licence, passport, student card.
When the forms are received, appointments will be made with a practice nurse and doctor if necessary.
In all cases, a patient registered with the practice can see any of the doctors by appointment.
If you are on long-term medication, you will be given a prescription re-ordering slip.
This allows you to request repeat medication without seeing the doctor every time.
Repeat medication is normally issued in a quantity to provide one or two months' supply.
After a specified number of repeats, you will be required to see the doctor or the nurse for review or any monitoring tests to ensure that all is well before further prescriptions are issued.
If you run out of medication we will do our best to issue you with a prescription as soon as possible.
If you print out any information from your record, it is also your responsibility to keep this secure.
If you are at all worried about keeping printed copies safe, we recommend that you do not make copies at all.
If you spot something in the record that is not about you or notice any other errors, please log out of the system immediately and contact the practice as soon as possible.
Routine consultations are by appointment with the doctor of your choice by telephoning 843677 (there are 4 telephone lines available) or you can book advance appointments On-Line.
We do not offer a service for "Walk-in" urgent appointments.
To improve access for patients with urgent problems we have appointments available, on the day, every day, subject to availability.
Appointments come available from 7.00am on line (for online access please register) and by telephone from 8.00am.
If you require urgent medical assistance which cannot wait until the surgery re-opens, please call the Out of Hours Service on 0161 763 8940.
The Surgery provides appointments outside the usual opening hours for a routine bookable appointment with Doctors and Nurses.
These appointments are available on Wednesday morning from 7:00am and Thursday evening from 6:30pm to 7:30pm.
The Bolton GP Practices have joined together to support 7 day access within Primary Care.
The seven-day service is being introduced to help patients fit their healthcare needs around family life and work commitments.
If you are registered with a GP in Bolton, then you can now book appointments to see a doctor or nurse at weekends.
