The Mote Medical practice is a five doctor, 9,800 patient practice in the south of Maidstone providing General Medical Services from its premises at St Saviours Road. We run many clinics for the management of chronic diseases such as asthma and diabetes and offer a wide variety of other medical services including antenatal and postnatal care, minor surgery, childhood vaccinations and well-person check-ups.

This decision was made to enable practices to work safely, reduce the chances of infection and to triage all calls coming into the practice. As yet we have no update on when we should switch this option back on. We want to help you get the right medical assistance when you're ill, injured or have a long term condition. Going directly to the person with the appropriate skills is important.

This can help you to a speedier recovery and makes sure all NHS services are run efficiently. Owned and run by the NHS, the NHS App is a simple and secure way to access a range of NHS services on your smartphone or tablet - including prescriptions and appointments.

You will appreciate that health data relating to any individual is highly confidential and the Practice must ensure that it releases such data only to the person to whom it relates, or to a person authorised to act on his or her behalf.
The General Data Protection Regulations (GDPR) gives you the statutory right of access to any information, manual (paper) or computerised.
You may wish to authorise someone else to make your application on your behalf and if you have parental responsibilities you may make an application to see your child's notes.

The government is encouraging general practices to establish Patient Reference Groups (PRGs) to help practices decide priorities to focus on to improve patient care.
We intend to appoint eight adult patient representatives of the community who will communicate with each other and the practice primarily by email to keep time demands to a minimum.
