Please do not submit urgent requests via Engage Consult, our online consulation service. Any patient of the Practice can use our online consultation service from a phone, tablet or computer to ask us a question 24 hours a day. Services
We provide a wide range of NHS services, from everyday treatments and clinics, to many specialist areas.
Where applicable, these services are provided by appointment during surgery opening hours.
You can now see a specialist physiotherapist (FCP) at Two Steeples Medical Centre instead of the GP.
This gives you faster access to expert diagnosis and you'll leave with the advice and exercises you need to start tackling the problem.
The physio can also refer you on to hospital services where necessary, order tests and write prescriptions.

If you would like to register with Bushloe Surgery, we would ask that you call into the surgery and collect a registration form and GMS1 form.
Alternatively you can download both forms below.
Some form of documentation with your current address, such as a current utility bill or bank statement.
Two blood pressure readings - we have a machine at the surgery for you to take your readings and attach to your registration form.
Once all your details have been entered onto our patient record computer system and your registration is complete, you will receive a confirmation letter enclosing details of how to access our online services.

If you are worried it may not be safe to wait for assessment in that clinic then you should call 111 who may advise you to attend A&E.
We have an established PPG to help us consult with patients on the facilities and provision of services.
We meet at least 6 times per year in the surgery at 6.00pm in the evening.
