Patients are encouraged to book an appointment with our diabetes specialist nurse or their GP if they have any concerns at any other time and not to wait for their annual review.
Services
Please note that our reception is open in the building at these times but our phones are only open 8:00am to 6:30pm Monday to Friday.
We are of course able to fulfil many requests through our website which is open all the time.
This includes appointment booking, checking results, ordering repeat prescriptions and requesting reports and sick notes.
We offer a full clinical service, including same-day assessment, between 8:00am and 6:30pm Monday to Friday.
If you need clinical assistance outside of these hours then please refer to the "When we are closed" section of our website.
Pharmacists dispense your prescriptions and other medicines, offer testing and screening for common medical conditions and can advise if you have a minor illness.
They can also help you decide whether you need to see a doctor or nurse.
You can talk to your pharmacist in confidence, even about the most personal symptoms and you don't need to make an appointment.
Most pharmacies now have a private consultation area where patients can discuss issues with pharmacy staff without being overheard by other members of the public.
Repeat prescriptions are medications which appear on the right hand side of your prescription and in your online profile which your doctor would like you to continue on a regular basis.
Medications not shown here are not repeat prescriptions.
You may hand in your computer slip or a note with your name, address, DOB and the medication required written clearly.
Repeat prescriptions will be ready in 3 working days from your request.
Please note if your prescription request is NOT for a repeat prescription it may not be possible to agree to your request, it will take longer to process and the doctor may wish to speak with you.
Our doctors work in Clinical Teams to provide continuity of care to patients.
They are supported by Team Care Coordinators who provide administrative support to the teams and are a point of contact and support for patients as well as ensuring proactive care by scheduling in activity and patient contact for the clinical teams.
All our doctors take part in our Same Day Team rota.
Each day we have at least 2 doctors forming the Same Day Team all the time between 08:00 and 18:30 and they are there to filter requests for same day appointments, provide telephone advice, provide face-to-face appointments for those who need to be seen, oversee home visit requests and deal with any patients who need an appointment soon yet are having difficulty accessing this through our system.
Our Patient Participation Group is composed of a small group of patients and staff from The Nelson Medical Practice who work together to foster an approach of partnership amongst all of those who work and use the services that are provided by the practice.
The PPG's objects are to represent the needs, concerns and interests of patients by consulting and working collaboratively with the Nelson Medical Practice about the services and facilities provided.
Acting as a "sounding board" for doctors and surgery staff, and to be a two-way channel of communication in the provision and development of healthcare services.
