During the current situation we request that you call the surgery for your condition to be assessed by a clinician - who will then, if required, contact you to discuss your ailments. Please contact your local pharmacy for minor ailments and your optician if you have concerns relating to your eyes. The normal Practice opening times are Monday to Friday from 8.30am to 5.45pm and operates an Extended Hours Service where GPs will use telephone consultations to contact patients until 7.00pm.

Medical assistance can be requested by calling 01355 573790 during Surgery opening hours. The Health Centre can be accessed by wheelchair and there are accessible toilets on every floor. New patients are welcomed by the Practice, please try to order at least one months worth of any medication you are taking from your previous practice.

Registration forms can be downloaded from 'Registering with the Practice' page on this site, please return ASAP as the practices based within the Health Centre operate a weekly rota for registrations and delay in receipt may lead to us deferring your registration.

A variety of vaccinations and boosters are given from two months of age, the only reason for not getting these injections are if your child has an acute illness or has had a reaction to a previous vaccination.

A range of vaccinations are available.
If in doubt please check with the Practice Nurse.
Influenza vaccinations are available each Autumn for the elderly and those with medical conditions such as asthma, diabetes, heart disease or kidney disease.
If possible also add copies of your ID or proof of your address.
Please obtain at least 1 months supply of any medication that you are currently taking from your previous practice as we may not be able to issue any medication whilst in the process of registering with us.
Patients are advised to the read the NHS counter fraud declaration before signing the registration form.
There are a number of reasons why you may not be able to register with your chosen GP, e.g.
Once you've been accepted as a patient, your medical records will be transferred to the Practice and you'll be sent a new medical card.
You can now order your repeat prescription online which many patients find much more convenient.
Repeat prescriptions may be requested online or requests may be deposited in the repeat prescription box at the Practice.
It would be of assistance if all repeat prescription items could be ordered at the same time.
The prescription takes two working days to process so please do not allow your medicines to run out.
Alternatively, you may arrange with the Pharmacy of your choice to deliver the re-order form to the surgery on your behalf.
When travelling please allow as much time as possible to arrange your travel appointment, with the Practice.
This will ensure that you are more likely to be fully vaccinated before you depart to your destination, we advise you attend to arrange your appointment at least six weeks before you travel.
Immunisation against infectious Hepatitis (Hepatitis A) is available free of charge on the NHS in connection with travel abroad.
