You can download a registration form from our "Registration" page or you can pop into the Surgery for more information and a registration form. TRAQS is a unique service for patients in Telford and Wrekin allowing you to be seen in the right place, first time.
If your GP decides to refer you for a consultation or treatment, TRAQS will contact you and manage the process.
TRAQS will ask you about your preferences - like the best dates and times for you and how far you are prepared to travel - and they will try and answer any questions you have and even help to arrange transport for you.

The receptionist may ask you for a reason for the appointment.
This is simply so you can get the most appropriate care as quickly as possible.
For example, our nurses are trained to deal with a range of issues.
